Free Choice of Provider Notification

Your Service Coordinator follows procedures developed from OAC 5123-9-11, Home and Community Based Services Waivers – Free Choice of Providers Rule.

If you are enrolled or in the process of enrolling on the Level One, Self-Empowered Life Funding (SELF), or Individual Options (IO) Waiver have the right to choose from any qualified provider, anywhere in Ohio, who are willing to serve you.

Things to Know:

• You may choose agency providers, independent providers, or a combination of agency providers and independent providers

• You may choose providers from all qualified and willing providers available in Ohio and are not limited to those currently providing services only in Warren County

• Providers may offer many services. Please know that you can choose to receive only one or more services offered by a provider

• A provider cannot require you to choose them for other services. For example, your residential or Homemaker/Personal Care provider cannot tell you that you must also attend their day habilitation program. Please contact your Service Coordinator if this ever happens

• If you choose the same provider to provide multiple services, you can choose a different provider at any time.

• Those choosing to receive Homemaker/Personal Care in a Residential Facility licensed by the Ohio Department of Developmental Disabilities are choosing both the place of residence and the Homemaker/Personal Care provider. However, you maintain Free Choice of Provider for all other Waiver services and the right to move to another setting at any time if a new Homemaker/Personal Care provider is desired
